For Your Stay
What to Bring
- A list of your medications including dosage
and times. If you bring your medication
bottles instead of a list, send the medications
home with your family AFTER the
doctor or nurse has reviewed them.
- A list of your allergies.
- Advance Directives about your healthcare.
- Any forms the doctor gives you to bring.
- Toiletries: toothbrush/toothpaste, shampoo,
comb, etc.
- Your glasses or contact lenses and supplies.
- Robe, non-skid slippers, and loose
fitting clothes.
- Your photo identification, insurance and/or
Medicare cards, required forms or deposits.
- Any assistive device (cane, walker, etc.)
that you currently use. Please label with
your name and address.
What to Leave at Home
- Any electrical appliances, including
computers and hair dryers. They are
fire hazards.
- Valuables: please leave all valuables
at home. Community Memorial Hospital
can not assume responsibility for loss
or damage to your valuables (jewelry,
money, etc.).
- Hairspray, gels, mousse, etc. should be
avoided prior to surgery.
Advance Directives
At Community Memorial Hospital, we believe
that each person has the right to accept or
refuse any medical treatment. Each person also
has the right to complete an Advance Directive.
An Advance Directive allows you to put your
healthcare wishes and values in writing, as well
as designate someone to make medical decisions
for you if you are unable to do so yourself.
You may also ask your doctor to complete a
POLST form, which clarifies your wishes for
end-of-life care.
If you have already completed an Advance
Directive or POLST form, it is important that
you bring it to the hospital so that it can be
kept with your records. If you have questions
about healthcare decisions please talk with
your doctor, our Social Services Department at
805/652-5010 or our hospital Ethics Committee.
