What to Leave at Home

  • Valuables: please leave all valuables at home. Community Memorial Hospital can not assume responsibility for loss or damage to your valuables (jewelry, money, etc.).
  • Hairspray, gels, mousse, etc. should be avoided prior to surgery.

Advance Directives

At Community Memorial Hospital, we believe that each person has the right to accept or refuse any medical treatment. Each person also has the right to complete an Advance Directive. An Advance Directive allows you to put your healthcare wishes and values in writing, as well as designate someone to make medical decisions for you if you are unable to do so yourself. You may also ask your doctor to complete a POLST form, which clarifies your wishes for end-of-life care.

If you have already completed an Advance Directive or POLST form, it is important that you bring it to the hospital so that it can be kept with your records. If you have questions about healthcare decisions please talk with your doctor, our Social Services Department at 805/652-5010 or our hospital Ethics Committee.
