NEWBORN INFORMATION (DOWNLOAD PDF)

Congratulations!
We share in your joy on the birth of your baby and hope your stay with us will be most pleasant. Thank you for allowing us to share in this special time. Enclosed are some important forms we would like you to read and complete. The forms will be picked up in the morning, so we hope you will be able to get started on them as soon as possible.

Souvenir Birth Certificate
If you would like a Souvenir Birth Certificate (not a legal document) that includes your baby's footprints, please complete the Birth Certificate Information Sheet and attach $3.00 to cover the expenses. The Auxiliary will collect and complete this certificate.

Live Birth Worksheet
Please read and complete the Live Birth Worksheet as soon as you have chosen a name for your baby. This will enable us to provide the information required to complete your baby's official Birth Certificate. If you are not married to the father of the baby you will need to fill out paternity papers, per California state law, to have the father's name on the birth certificate. Our Birth Clerk will pick up your Live Birth Worksheet, and discuss paternity papers with you, if necessary.

Hearing Screen Consent
You will be offered a hearing screen for your baby. Please read over the enclosed consent. A Hearing Screener will stop by to discuss the screen with you.

Newborn Screen
Per California law, all babies are screened for certain genetic conditions soon after birth. Drops of blood will be taken from your baby's heel, and then sent to an outside laboratory. The results will be sent to your pediatrician's office. The Newborn Screen Booklet explains the test in more detail.

Feeding and Diaper Record
Please fill in the times you feed your baby, how long your baby breastfeeds, or how much formula your baby eats, and mark a check in the appropriate box for any diaper changes. Nursing Care
The mother-baby nurses are always available to answer your questions and to provide instructions or demonstrations on everything from breastfeeding to care of the umbilical cord. Simply ask the nurse for assistance by using your call light.

Infant Care
Supplies For your convenience, there are diapers, wipes, t-shirts, and blankets in the infant bassinets. Simply wet the wipes with warm water to use. Ask your nurse, or stop by the nursery, if you need more supplies. Upon discharge, please take supplies with you, but leave the linens (blankets and t-shirts).

Hand Washing
We ask that all visitors wash their hands well prior to handling the baby. Please help us instruct your family on the importance of good hand washing, as this is the most important aspect of preventing infection in your baby. You may use the soap provided in your bathroom dispenser.

Newborn Photos
A souvenir baby picture will be taken by a baby photographer. The photographer will give you information regarding ordering pictures.
