Seminar to Focus on ‘Five Wishes’ Documents for Cancer Patients

Community Memorial Hospital’s Psychosocial Services Department is holding a seminar on Monday, Oct. 17, to provide cancer patients “Five Wishes” legal documents that puts in writing their healthcare wishes to family and loved ones.
Jody Giacopuzzi, a Licensed Clinical Social Worker, will facilitate the seminar that will run from 5 to 6:30 p.m. at CMH’s Cancer Resource Center, 2900 Loma Vista Road, Suite 105.
A “Five Wishes” document can help family members talk with each other about difficult health care subjects. It addresses:
- The person who will make care decisions when the patient cannot;
- The kind of medical treatment wanted and not wanted;
- How comfortable the patient wants to be, including pain management;
- How the patient wants to be treated;
- What the patient wants loved ones to know.
The Durable Power of Attorney for Health Care and the Physician Order for Life Sustaining Treatment documents also will be discussed.
