About
Community Memorial Health System offers an
Industrial Health Program specifically designed to
meet your growing needs as an employer.
High quality, cost effective and convenient healthcare services are provided to employers and their employees. To meet these needs we have developed a program that will help minimize lost time from work as well as decreased time on modified work status. Our Industrial Health program works with employers to ensure that employees receive high quality care and return to work as soon as medically indicated.

Communication
After each initial injury visit as well as all subsequent
visits to our facility employers and patients promptly
receive copies of the appropriate documentation. This
documentation includes work status, a description of
the patient's condition, work restrictions, follow-up
recommendations, referrals and appointments. Open
channels of communication are encouraged and contact
information is also included with the documentation.
Additionally, every center is staffed with an Industrial
Health Representative to serve as a point of contact for
employers and patients.

Materials Provided
You will receive Industrial Health Program identification
posters with contact information, a map and
directions to each facility. Referral and Authorization
Forms are also included. The Referral Authorization
Form should be brought with the patient to the facility.
A supervisor or responsible party at the job site should
sign the form.
