Community Memorial Hospital is pleased to offer our patients a pre-registration service that allows us to verify all demographic and insurance information prior to your delivery. Expectant mothers should pre-register by the seventh month of pregnancy. The pre-registration process allows CMH to communicate with your insurance company to verify benefits and eligibility, and eliminates the stress of doing this additional paperwork when you come in to deliver your baby.

You can pre-register by coming to the Admitting Department on the first floor at CMH, at the conclusion of the Maternity tour, or pre-register on-line here ▶

Please also complete the birth certificate worksheet and bring it with you when you come to the hospital to deliver your baby.

The Admitting department is open from 6:00 a.m. to 9:30 p.m. daily.

You need the following information to pre-register:

  • Name of the Pediatrician/Family Practice Physician who will care for your infant
  • Insurance company information/card
  • Name of your employer
  • Admitting date/due date
  • Social Security Number
  • Emergency notification numbers

Please bring identification that includes your photograph and signature (such as your driver’s license) when you come into the hospital.
