Health Information Management at Community Memorial Hospital is responsible for securely maintaining the medical records of patients who have received care within our organization. In order to comply with state and federal law, we may only release a copy of a medical record with the proper authorization from the patient or a designated representative.

Physician

You may also choose to have your physician call and request documentation necessary for an appointment ahead of time. This will ensure any necessary reports, results and/or documentation is available at the time of your visit. Medical Records Fax: 805/948-5649.

COVID-19 test results may be obtained from the physician who ordered the test.

In Person

In order to ensure patient and staff safety during the COVID-19 pandemic, CMHS is restricting in-person access to the Medical Records department.  Please utilize the listed options on this website to request a copy of your medical records. If you are unable to utilize any of the prior suggested options to obtain your medical record please contact the Medical Records department for an appointment at 805/948-5047.

Delivery Options

After your request has been processed and is complete, we will deliver the documents in the requested format and requested mode of delivery. Please be advised that when utilizing email there is some level of risk that your requested health information could be read or otherwise accessed by a third party while in transit.

The HIM Department will be closed to the public on the following major holidays:

  • Presidents Day
  • Memorial Day
  • Independence Day
  • Labor Day
  • Thanksgiving Day
  • Christmas Day

To protect patients’ privacy and health information, we do not accept telephone requests or disclose patient information over the telephone.

Request to Amend Your Record

  1. Please contact Medical Records for a current copy of the “Request for Amendment of Protected Health Information” form: 805/948-5047
  2. Complete and sign amendment form and return via mail, fax or email.

Please note: unsigned or incomplete requests cannot be processed.

Please allow 7-14 workdays prior to contacting the Medical Records department to request a status of the request.   

Request an Accounting of Disclosures

  1. Please contact Medical Records for an “Accounting of Disclosures” form: 805/948-5047
  2. Complete and sign Accounting of Disclosures form and return via mail, fax or email.
